Managing Telephone Services in a Department

U.Va. Telephone Service Request (TSR) Forms

An on-line electronic form is available for standard on-Grounds' telephone service requests. Login to the e-forms system using the middle blue section of the e-forms login screen. Next, select "Blank Forms." Finally select "Communication Services" and then select the "Communication Services Req CC40 form."

Forms for other services are available from the Communication Services business office at 924-4148 or by visiting the office in Carruthers Hall.

Forced Authorization Code (FAC) Requests

A Forced Authorization Code (FAC) will allow users to place long distance calls from within the University area and bill those calls to a specific PTAEO. To apply for a FAC, print the Request Form, and submit the completed form to the Communication Services window in Carruthers Hall, or mail it to ITC Communication Services, P.O. Box 400196. This same form can be used to request a University Calling Card.

Paging Access Form

This paging access form is used to add or remove a person or function from the Paging Database or change a person's information. When you click on the "Submit" button, a form with the information you entered will be displayed. If the form is correct, print it, add any remaining information and send it to P.O. Box 800290. If you wish to make changes, use the "Back" button. The form is available at: http://www.itc.virginia.edu/commserv/paging/pager.php.

Telephone Equipment Choices

There are several models of the ROLMphone available at U.Va., each of which can be equipped with data. The fundamental difference between a data phone and a voice-only version of the same model is that the voice-only model lacks:

  • the ability to transmit data when connected to the appropriate computer workstation or terminal
  • an RS232-C connection to the phone
  • an electrical power outlet on the phone

Answering machines will not work with the digital connections. PhoneMail is available as an alternative to answering machines or you may install a ROLMphone 612 or 624 with an analog adapter.
